If you need a form, for whatever reason, I can recommend formspring.

I’ve created a sample form here.

You can sign up for free, and have 3 forms, but there most popular package is only $19, and that integrates PayPal which would be really useful if you were selling something (watch this space!). You can build the forms yourself, or use a pre-built template- it is very easy to use and intuitive. On the paid packages (professional and above), you get full SSL encryption - it is very easy to use and intuitive. Any data which is inputted is stored and can be looked at any time - here’s what they say on their website:

* Create Saved Searches and make browsing and finding your data easier.
* Use the FormSpring API to add, edit and retrieve data stored within your FormSpring account.
* View summarized reports of submitted data.
* Download submitted data as a CSV file to use within Microsoft Excel or Access.
* Download submitted data as a RTF file to use within Microsoft Word.
* Receive your form submissions with an RSS subscription.
* Share data and reports with other users without giving them access to your account.

Overall, I am very impressed, and would certainly think about using them in the future. Yes, contact forms can be easily integrated for free in blogs and websites, but having that PayPal functionality is nice and being able to search all the inputted data is also a big plus point.
